How to retrieve the form’s URL
You can get the URL of your Microsoft Form by clicking on “Collect responses” and “Copy link”.

Alternatively, if you are navigating the form as a viewer, you can copy the address displayed in your web browser’s address bar.

If you are copying the URL from the address bar, make sure that you are NOT in edit mode, otherwise the form will not be embedded correctly. |
Retrieve the response summary link
If you want to embed the response summary in your Confluence page you’ll need the summary link.
Go to Responses tab, click on the option button and then on Get a summary link.
Copy it in your clipboard and paste it in your Confluence page.
